Application Instructions (Please Read)

1. Read the Hints for Developing Research Proposals.

2. We require a paper copy, with all the necessary signatures included, mailed to the SBC coordinator in your region. See #7 below for printing instructions.

3. Fields with an asterisk (*) are mandatory before submitting your application online. All appropriate signature fields are mandatory on the hard copy version.

4. Please see the Regional Competitions web page for your regional for application deadlines and regional coordinator mailing address.

5. Your proposal will be assigned an ID number which will be emailed to you by your regional coordinator.

6. You may complete this application in more than one sitting. Click Save my progress and resume later at the top or bottom of the application. At the top of the application you will see a place to enter your email address and the password you choose. Save the application. Write down your email address and password. You can return at any time to the same internet address to resume your application.

7. When ALL of the fields have been filled in, print your application (Ctrl+P on PC or Cmd+P on MAC) and THEN click Submit. You will be given a chance to review your data, edit it if necessary, print it, and then make your final submission. If you do not receive notification at the top of the application that you can now edit, modify, and print your application, it means that your application is incomplete. Review your application. Any errors will be highlighted in red. Correct them and print your application.

8. After you check your data, make any necessary corrections, and print your application, click Submit again. You will be taken to a confirmation page that confirms that your application has been submitted. If you do not see this confirmation page your application has not been submitted. Return to the application and correct any errors and submit again until you receive confirmation that your application has been received.

9. Supervising teachers must read and sign off on the proposals after confirming that they are within the SBC Rules & Project Guidelines.

10. Take the printed copy, obtain the appropriate signatures (teacher, principal, parent/guardian, applicant), and mail it to the regional coordinator for your region. Failure to submit the signed copy constitutes an incomplete application and will result in the disqualification of your project.

Declaration, Discharge & Release of Information

Section A: The Sanofi Biogenius Canada (SBC) rules are available from SBC Regional Coordinators and on the SBC web site. All projects must comply with these rules. Projects failing to do so may be subject to sanctions, which may include disqualification.



 

Section B: Participants (under 18 years of age) - Authorization, release and indemnification

Permission is hereby given for my son/daughter/ward to travel to and participate in the upcoming Sanofi Biogenius Canada. I hereby release and forever discharge the members of the Organizing/Management Committee and all their agents, officers and employees from all manner of legal actions, claims or demands which may arise from or be caused by attendance of the child at the SBC competition. I further agree that should the child be the cause of any damage, loss or injury to another person, or prejudice to any organization while attending the SBC competition, I will indemnify the members of the Organizing/Management Committee. I also understand that the child will be disqualified and sent home immediately, at my expense, if he/she does not behave as expected with due respect for the rights, health and safety of other participants and the general public, or if he/she behaves in an offensive manner. 



 

Section C: Participants (18 years of age or older) - Declaration, release and indemnification I hereby release and forever discharge the members of the Organizing/Management Committee and all their agents, officers and employees from all manner of legal actions, claims or demands which may arise from or be caused by reason of my attendance at the SBC competition. I further agree that should I be the cause of any damage, loss or injury to another person, or prejudice to any organization while attending the SBC competition, I will indemnify the members of the Organizing/Management Committee. I also understand that I will be disqualified and sent home immediately, at my expense, if I do not behave with due respect for the rights, health and safety of other participants and the general public, or if I behave in an offensive manner.



 

Section D: Release of information

Pursuant to matters of freedom of information and protection of privacy, I hereby give permission to the SBC to release my name/the name of my son/daughter/ward, his/her picture and/or videotape, as the case may be, for publication in the media, including the World Wide Web, or to sponsors or prize donors. In compliance with the aforementioned, the SBC shall not release personal information to any organization other than those specified previously. Except for the Organizing/Management Committee, no one may film or photograph any project whatsoever without the express permission of the Sanofi Biogenius Canada participant. Since pictures and films will be taken by the SBC during the SBC competition, I hereby give them permission to use those pictures and films in whole or in part for publicity purposes in brochures, magazines, newspapers, television, the World Wide Web, etc. All said material, whether used in whole or in part remains at all times the property of the SBC. 



 

Section E: Consent

I consent [or I consent on behalf of my son/daughter/ward] to the collection of personal information about me [or my son/daughter/ward] by the SBC regional coordinator and the disclosure of that information to Partners in Research, the SBC national coordinator, and to the SBC sponsors. The personal information will be confined to my [or my son's/daughter's/ward's] name, address, telephone number, project name, year and location of participation, awards, post-secondary field of study and institution and my [or their] rating of the SBC's influence on my [or their] career plans. The personal information will always be stored in a password-protected database. Partners in Research and the sponsors will use this personal information to assess the success of the SBC program.

Discharge and Release of Information for students under 18 years of age

I have read the “Rules of the Sanofi Biogenius Canada” and my child/ward agrees to comply with the Rules. My child/ward agrees to abide by the decisions of the Evaluation Committee, whose decisions will be final and without appeal.
